In order for us to start the set-up process, we will need the following items.

• Company Name you would like to display
• Service Provider's Full Name (If different)
• Brief company bio (what you'd like us to say about you)
• email address
• Mobile Telephone Number
• Are you mobile? have premises? or both?
• If mobile, do you have a call-out charge?
• Day by Day opening hours
• Price List with duration of service etc (send as image, document)
• Any terms and conditions you would like adding to your booking page.
• Any specific logos and/or images you would like to use to represent your brand and your services (otherwise stock images will be used for services)

Most of the above can be uploaded using the "attach document" button at the bottom of the form, However, if you have anything in a format not accepted by this form, then please email to admin+onboarding@effortlessvip.com
